Cardiology opening in Fairfield, Connecticut. We are actively seeking a Nurse Practitioner or Physician Assistant to join an established group, providing primary care in Fairfield County. Must have interest or experience in Bariatric Medicine. Overview:

• No New Grads
• Must have at least 1 year of Primary Care or Bariatric experience
• Flexible schedule: 4-day work week Compensation & Benefits:
  • Salary $120k-$140k/year

  • 401k with employer match Job Responsibilities:
    • Examine, obtain medical history, and treat acute and chronic illnesses.
    • Order, perform and interpret diagnostic tests.
    • Create treatment plans and involve patients and family members in those plans.
    • Initiate appropriate specialty referrals.
    • Collaborate on improvement in care delivery and service with management, other physicians, and staff.
    • Work with care team to close care gaps and improve quality measures.
    • Participate in Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives.
    • Work with practice management to ensure compliance with billing and coding requirements.
    • Provide on call coverage within the primary care group, approximately 1 in 8. Education/Licensing Requirements:
      • Graduation from accredited APRN or PA program.
      • Valid and unrestricted Connecticut license.
      • Valid DEA registration and Controlled Substance Certificate.
      • Minimum of 1 year experience in primary care required.
