Flexible scheduling Work-life balance with home-based visits Strong support from a dedicated interdisciplinary team Serve your local community with culturally informed care Opportunities to work with diverse populations including pediatric, adult, and geriatric clients Engage in meaningful, one-on-one patient care Gain valuable experience in home health — a growing and essential field Job Details Evaluate clients' physical functioning in their home environments Develop personalized treatment plans with defined goals and visit schedules Implement care plans under physician direction and in collaboration with agency staff Coordinate with clients, families, and caregivers to support rehabilitation efforts Observe, document, and report patient progress and any changes to physicians and team members Collaborate with other professionals to establish rehabilitation and maintenance activities Provide care across various age groups, with a focus on pediatric populations Skills You Need Current New York State Physical Therapy license and registration Minimum one year of PT experience in hospital or long-term care settings Pediatric experience required; multi-population experience (adult, geriatric) a plus Ability to travel to patient homes using public or private transportation, including in challenging environments Interested in hearing more?
