Contract | Travel / Per Diem Location: Fairfield, CA Schedule •
Saturdays only • 8:00 AM – 4:30 PM • 8 hours per week Contract
Duration: Approximately 1 month Pay Rate: $40.00/hour About the
Role Cornerstone Staffing Solutions is partnering with our client ,
a healthcare organization, to identify an experienced Emergency
Department Case Manager (RN) for weekend coverage. This role is
heavily focused on utilization review , care coordination, and
ensuring appropriate patient transitions across the continuum of
care. The ED Case Manager works collaboratively with physicians,
nursing staff, social services, and interdisciplinary teams to
support optimal patient outcomes while aligning with payer
guidelines and regulatory requirements. Key Responsibilities •
Perform utilization review using MCG criteria • Assess
appropriateness of admissions, level of care, and continued stays •
Coordinate patient transitions and post-ED care planning •
Collaborate with physicians, social services, and interdisciplinary
teams • Advocate for patients and families throughout the care
episode • Document assessments, utilization data, and discharge
planning in Cerner • Communicate with health plans regarding
authorizations and coverage • Coordinate facility transfers and
outpatient referrals • Identify high-risk patients and support
readmission prevention efforts • Ensure compliance with federal,
state, and local regulations • Utilize SBARQ for effective care
transitions and handoffs Required Qualifications • Active
California RN license • Bachelor of Science in Nursing (BSN) •
Minimum 3 years of acute care nursing experience • Minimum 2 years
of case management experience • Strong utilization review
background (MCG) • Proficiency with EMR systems (Cerner preferred)
• Solid understanding of Medicare regulations and Conditions of
Participation Preferred Qualifications • Emergency Department
experience • Experience with MCG and Cerner • Certified Case
Manager (CCM) or Accredited Case Manager (ACM) • Master of Science
in Nursing (MSN)
