This full-time role has coverage 4 days per week in Manhattan where you will see patients in the SNF, Hospital and community settings. The fifth day will be REMOTE Working from home office where there will be a Virtual Hospice Team IDT meeting, and you will do telehealth follow up visits. NP should have at least 2 years experience in either Hospice, Palliative Care The Nurse Practitioner will provide direct skilled nursing care to patients in the nursing home, hospital or community setting and will manage pain and symptom management, communication including goal setting discussions, and hospice eligibility review. NP will do patient assessment – both cognitive and physical, and may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family. NP will be responsible for taking patient history, do appropriate examination / patient assessment, create and implement a plan of care to prevent or mitigate illness and not burden for the patient and family. Orders appropriate diagnostic testing Formulates a problem list Recommends interventions consistent with a care plan that advances the objectives of palliative care Provides interventions as appropriate, consistent with a care plan that advances the objectives of palliative care If appropriate, takes the lead in implementing key elements of palliative care, including family meetings, goal setting discussions, and symptom management. Performs hospice eligibility review and engages in accurate hospice-related discussions Collaborates with patients’ physicians and other treatment team members Evaluates patient's response to treatments, modifies or changes when needed.
