Ensures adequate staffing and appropriate patient assignments. Maintains the integration of interdisciplinary services. Facilitates the achievement of agency goals related to clinical outcomes, patient experience of care, fiscal responsibility, and staff retention. Mentors and develops nursing staff in the provision of patient care and services that are aligned with agency guidelines and policies as well as all applicable state and federal regulations. Working collaboratively with the Clinical Director, identifies educational opportunities to facilitate the ongoing development of clinicians related to the delivery of community health nursing. Oversees & evaluates performance of individual COCs, CHNs, LPNs, related to their ability to deliver, accurately document, effectively coordinated quality driven & evidenced based care to clinically complex patients in accordance with the physician established plan of care Creates an action plan in collaboration with the clinician when indicated to improve clinician’s performance. Monitors clinician’s progress towards action plan goals and evaluates clinician’s ability to maintain improved and sustained performance. Evaluates clinicians individually and collectively as a team in their ability to provide care and services in a manner that facilitates the achievement of improved patient experience of care scores as reported by Press Ganey. Provides oversight and direction related to the scheduling of patient care services by the appropriate level of clinician to ensure high quality care delivery, continuity of care, and staff productivity. Reviews and processes various reports to assess staff adherence to agency stated workflows. Any noted variances are acted upon in a timely manner to improve performance and avoid future deviations from established guidelines and workflows. RN MUST have OASIS / clinical outcome measures. Education BSN / Master’s degree preferred. Experience Minimum of 3 years’ experience in a Certified Home Health Agency; supervisory experience preferred. Interested in hearing more?
