Job Description:
Location: Long Island or Bronx, NY About the Role: Seeking
experienced specialty physicians to provide consultative care for
residents in nursing home settings. This role focuses on
telemedicine consultations, with occasional in-person visits,
particularly for initial assessments. Specialists in Psychiatry,
Cardiology, Endocrinology, Nephrology, Neurology, and Infectious
Disease are encouraged to apply. Key Responsibilities: Provide
telemedicine consults for nursing home residents in your specialty.
Conduct initial in-person assessments as needed. Collaborate with
nursing home staff, primary care providers, and other specialists
to develop and implement individualized care plans. Review patient
histories, diagnostic tests, and lab results to provide expert
recommendations. Ensure timely documentation of consults and
recommendations in the medical record. Qualifications:
Board-certified or board-eligible in your specialty. Active NY
medical license or eligibility for licensure. Strong communication
skills and ability to collaborate in a multidisciplinary team.
Prior experience in a nursing home or long-term care setting is a
